

Mailing Requisition Form
In an effort to insure that all of the mailings you send to Central Services are processed as quickly and accurately as possible, we’re asking that anyone who is sending a mailing job to us please fill this mailing requisition form.
When possible, it would be helpful to send a copy in advance of a mailing to make us aware one is coming and allow us to schedule accordingly. Also please attach a corresponding copy of the requisition form to any materials that you send to our office. (Note: if you fill out the form online, it will e-mail you a copy of what is submitted; you can print out the e-mail to attach to your materials). We often have many inserts, letterhead, and envelopes to several different jobs brought to us from the same office; clearly marking each box of materials with the jobs name and contact person would greatly reduce the margin of error. We would also appreciate a sample of your mailing whenever possible.
