Purpose
The questions below are intended to elicit information from you about your overall wellness in a variety of areas and provide you with feedback to assist you in achieving success at college.
Once you complete all the questions, click on the box "Get my results." You will receive individual feedback for each area. Should you need documentation that you completed the Wellness Quotient questions, click on the print icon and a Certificate of Completion will be generated. Place your name on the Certificate of Completion and return it to the Office of Alcohol and Other Drug Programs. The staff there will log only your name and unique identifying number on your behalf and notify the specific department that asked you to complete the online Wellness Quotient.
Confidentiality
The information you provide is anonymous and no identifying information is attached to your responses. The hope is you will answer these questions openly and honestly, which when you finish will give you a better idea of how you are progressing toward your overall wellness.
